INSTRUCTIONS FOR FORM SUBMISSION:

Email as an Attachment

• Download the Questionnaire form at:
  https://www.theforgottenfoods.com/media/docs/Consultation-Questionnaire.doc (right click, save as)
• Open the file in WORD or another word processing program
• Complete all form fields
• SIGN (digital signature is accepted) enter the submission date, print your full name, and enter a 5-digit pin number
• SAVE the file (be sure you save your questionnaire form often so no information is lost)
• EMAIL as an attachment to: longlife@theforgottenfoods.com
• Your email SUBJECT line should read: Consultation Questionnaire - (your full name)

Print and Fax

• If you are not able to use the Word format (will not work with OpenOffice), download the PDF version:
